Overview

The Nurse Case Manager will provide case management services to an ongoing case load of individuals living with HIV/AIDS.  The RNCM works as part of an interdisciplinary team to formulate care plans based on assessment and provision of care priorities. Coordination of services with the social worker /mental health clinician, and other psycho-social support services for persons with HIV/AIDS needing higher level of care. The RNCM will work under the Home Based Case Management (HBCM) program or the Medical Care Coordination (MCC) program. The RNCM for HBCM will coordinate services for persons who are homebound and when applicable, will coordinate activities with in-home support, skilled nursing facilities, hospital nursing personnel, members of the interdisciplinary team and with the physician in the clinic. The RNCM for MCC will act as team lead to ensure the patient’s needs are met and their care is coordinated through the delivery of brief interventions focused on patient education, treatment adherence, managing side effects, medical nutrition therapy, co-infections, and preventative care.

Responsibilities

  1. Identifies and enrolls persons with HIV/AIDS into a case management program providing services within the HMCM and MCC programs.
  2. Works as part of an interdisciplinary team to formulate care plans based on assessment and provision of care priorities.
  3. Coordinates services with the social worker /mental health clinician and other psycho-social support services for persons with HIV/AIDS needing higher level of care.
  4. Coordinate access to other services including, nutrition services, legal services, in-home support, attendant care, and skilled nursing.
  5. Actively participants in Case Conferences to determine appropriate care plan for patients.
  6. Delivers brief interventions focused on patient education, treatment adherence, managing side effects, medical nutrition therapy, co?infections, and preventative care.
  7. The Nurse Case Manager will participate in the developing of all program material, policies and procedures related to the nurse case management program; includes creating program manuals and Quality Management policies.
  8. Develops program informational and educational materials and various forms.
  9. Cultivates and develops community social services linkages for patients and families with HIV/AIDS.
  10. Identifies third party payer sources, completes Medi-Cal and/or Medi-Care paper work; Qualifies participants for the State AIDS Case Management Program services.
  11. Collects demographics, compiles monthly reports and statistics.
  12. Works to coordinate support services with hospital discharge planners as needed.
  13. Attends staff meetings, programmatic trainings/meetings, various community meetings, task forces and local consortium.
  14. May be required to perform clinical nurse duties as needed
  15. Performs other related duties as assigned.

Qualifications

  1. Graduation from an accredited Baccalaureate School of Nursing or a related field and two years experience in administration or management, public health nursing and/or case management or an RN with three years experience in home health care and/or case management.
  2. Current, active, and unrestricted registered nursing license provided by the State of California.
  3. A minimum of one year experience working with HIV populations preferred.
  4. Bilingual: English/Spanish required

RESPONSIBILITIES
This position is responsible for developing and facilitating individual services and group activities that will help encourage healthy aging and develop a sense of community among residents. You will work in conjunction with the Los Angeles Office HIV/AIDS Initiative team and provide comprehensive and clinically focused case management and support services to residents of the Palm View. In addition, you will plan and coordinate a variety of activities for the Palm View Residents as well as clients of The Actors Fund HIV/AIDS Initiative that builds upon and maximizes the creative skill set inherent in our population. This role is ideal for an organized, compassionate, creative and inventive Clinical Social Worker.

REQUIREMENTS
To interview for this position, you must have a Master of Social Work degree, LCSW preferred. We are ideally looking for someone with 2 years’ experience working with Disability and HIV/AIDS related issues and concerns, Mental Health, Chemical Dependency and Dual Diagnosis, as well as experience working with seniors and aging in place.

Online Support Group needs facilitator (LCSW or Psychologist)

Is anyone interested and/or do you know someone who may be interested in conducting this group for the Laurel Foundation?  a licensed Psychologist would be ok for them as well.

PS:  Even though the position is unpaid, remember it is only one day a month

We are in need of a California licensed Social Worker, who can volunteer their time once a month (day is flexible) from 8:30 PM – 9:30 PM to facilitate a video online support group.  The group is composed of five English-speaking mothers who are either HIV+ or have children who are HIV+.

We are looking for a commitment of one year.  So there would be 12 sessions (summer months and in December we usually do not have a session due to holidays etc.). The Laurel Foundation will do all the backend work (make sure all parents are reminded of the call) and be on call should any issues (technical or otherwise) arise.

JOB SUMMARY:
Under the supervision of the Psychosocial Case Manager Supervisor, the Psychosocial Case Manager serves as an integral member of the health and mental health services team by providing psychosocial case management and overall support services to patients while ensuring that psychosocial case management services operate within industry standards and meets all contractual requirements and guidelines.

ESSENTIAL FUNCTIONS:

  1. Provide benefits/social services;
  2. Develop comprehensive benefits/social services plans for clients, providing support & guidance to client in the implementation of services plans;
  3. Prepare and submit monthly transportations reports to OAPP Transportation Unit;
  4. Prepares and coordinates paperwork for public and private disability programs;
  5. Acts as a liaison between the clinic and Department of Public Social Services for Medi-Cal Applications;
  6. Maintains an active case load with clients requiring on-going case management services and maintains complete and accurate files on all clients and document services provided;
  7. Provide appropriate information and referrals while serving as an advocate;
  8. Network with other community agencies and organizations in order to establish referral linkages;
  9. Assists in developing and researching resources for case management clients;
  10. Attends and participates in weekly staff meetings, case conferences and other meetings as assigned;
  11. Act as a liaison between the clinic and Department of Mental Health Housing and Homeless Division;
  12. Present clients in weekly case conferences;
  13. Makes appropriate referrals to ancillary team members, e.g., nutritionist, nursing case management, etc.
  14. Keeps Psychosocial Case Manager Supervisor informed on all patient information, or concerns;
  15. Provides referrals for a housing match based on clients’ psychiatric condition, social support network, substance abuse issues, and readiness for independent living;
  16. Documents all services provided and maintains client files and program statistics as required by Agency, contract, and or other related agencies;
  17. Provides best match housing placement and assists client with interviewing for housing placement;
  18. Interfaces with Department of Rehabilitation and other related community agencies relative to housing for clients;
  19. Gathers/maintains available information and prepares routine reports related to housing placement activity including client placement, successes and challenges to determine trends within the housing market and ensure long term placement for clients;
  20. Consults with staff on service coordination and service plans for clients referred to ICM program;
  21. Stats current on housing market activity to identify trends and prospective openings;
  22. Keeps the ICM program staff apprised of new housing opportunities;
  23. Maintains accurate, complete documentation of services provided to clients, ensures compliance with Agency policies and procedures, state licensing, on other regulatory agencies, prepares a variety of reports on housing placement activity for program, division, and Agency;
  24. Develops and maintains housing placement skills through seminars, workshops, or other forms of training/education;
  25. Other duties as assigned.

JOB QUALIFICATIONS AND EXPERIENCE:

  1. At least 3 year experience providing direct client case management services, preferably within a clinic setting;
  2. Comprehensive knowledge of HIV/AIDS community resources, public and private HIV/AIDS benefits programs;
  3. Understanding of the global effect of HIV/AIDS on the person, their family members and significant others;
  4. Have a working knowledge of the housing market, DMH programs, and placement of individuals that meet the DMH criteria for placement services.
  5. Ability to handle multiple tasks, be detail oriented, and maintain strict confidentiality of medical information;
  6. Bilingual (Spanish/English) preferred;
  7. Superior verbal and written communication skills, including strong organizational, detail and interpersonal skills;
  8. Good computer skills and knowledge including word processing, database operations, spreadsheets (Microsoft Excel), and other software systems;
  9. Ability to work effectively with men & women of diverse races, gender identities, ethnicities, ages, & sexual orientations in a multicultural environment.

TRAINING, EDUCATION, AND LICENSURE:

  1. B.A. degree in social work, psychology, sociology or a related field is preferred;
  2. Current AHA CPR certification.
