HIV Mental Health Task Force Meeting 11/08/17, Actors Fund

Please join us for our monthly HMHTF meeting MHTF agenda 11-08-17 hosted by The Actors Fund Wednesday 11/08/17 from 9:30am-11:30am.

The Actors Fund will be able to validate parking for the following location: S.C. Prestige Parking: 5657 Wilshire Boulevard. Lot entrance is on Hauser, on the west side of the street, ½ block North from the corner of Wilshire. (Building entry/exit is located directly next door to Black Dog Café.) This is valet parking. We have arranged this service with the parking office and NO TIPPING IS NECESSARY.

Important Note About Parking: While they do validate as indicated above there are some people that choose to park at metered street parking in the area. If you choose street parking over these lots, please read all parking signs carefully, as restrictions apply. Most importantly, DO NOT PARK ALONG WILSHIRE BOULEVARD between the hours of 7-9am and 4-7pm (Monday – Friday) or your car will be ticketed and towed. This applies even if you are parked at a meter.

practice. Drawing on 27 years of experience in the field, we are

working to elucidate the varied factors that affect the health of

Hispanics/Latinos in the United States and U.S. territories, with

special attention to the ongoing scourge of HIV/AIDS. Through

research as well as social programs, the Commission is dedicated to

reducing new HIV infections, unravelling the social determinants

of health and identifying the unmet needs of Latinos living with

HIV infection”

Today, the Departments of Health and Human Services, Labor, and the Treasury are publishing FAQs that solicit comments on how to improve disclosure under the Mental Health Parity and Addiction Equity Act and other laws, as required by the 21st Century Cures Act. As part of the solicitation of comments, the departments are also releasing and seeking comments on a draft model form that participants, enrollees, or their authorized representatives could use to request information from their health plan about nonquantitative treatment limitations that affect their mental health or substance use disorder benefits.

Job Title: Case Manager III – HIV Services

Under the general supervision of the Mental Health Supervisor, the Case Manager III will be responsible for functions related to the Home-Based Case Management/Medi-Cal Waiver Program (HBCM/MCWP) or the Medical Care Coordination (MCC) program. This position will work as part of the interdisciplinary team in a primary medical care facility for persons living with HIV and AIDS. The scope of duties include but are not limited to: conduct psychosocial assessments and re-assessments of the patients enrolled in the HBCM/MCWP or MCC programs, provide social and community referrals to assist patient with accessing psychosocial services provide emotional/mental health crisis interventions, participate in case presentation with the RN Case Manager (RNCM) involved in care of patient conduct emergency referrals, document promptly and assist clients with advocacy and education about HIV and AIDS. For LCSW, clinical duties may be assigned which will include short-term therapy, crisis intervention, and other evidenced-based practices.

 

Responsibilities:

  1. Conducts initial comprehensive psychosocial assessments of new patients and periodic reassessment as indicated by contract protocols and any changes in client’s medical, mental and/or emotional status.
  2. Works closely with the RNCM and others from the comprehensive medical team to develop and implement comprehensive quality care plans.
  3. Provides appropriate referrals; acts as a liaison and advocate for client with other agencies, facilitation access to public assistance programs and other AIDS organizations, medical needs, mental health services, housing, and any other care plans.
  4. Provide crisis interventions and/or other interventions within the scope of practice. Assesses clients for indicators of any acute and/or ongoing psychological distress and/or substance abuse.
  5. For LCSW, provide age-appropriate and culturally sensitive counseling and individual/family treatment as well as assistance in family life crisis situations.
  6. Attend and participate in program meetings, including weekly multidisciplinary team meetings. Consultation and ongoing communication with the other members of the multi-disciplinary team about the client and result of evaluation and interventions.
  7. Provide counseling/education to client and his/her family
  8. Prepare, organize and maintain client’s chart.
  9. Keep track of changes on state forms and consents.
  10. Maintains and completes accurate and timely documentation.
  11. Maintains a case load of 40 patients(HBCM/MCWP program only).
  12. Keeps abreast of latest medical information regarding HIV and AIDS.
  13. Attend weekly clinical supervision (group and individual) in accordance with regulatory board requirements.
  14. Provide coaching and mentoring to Case Managers I & II as needed.
  15. Perform other related duties as assigned.

 

Qualifications:

  1. Masters Degree in Counseling Psychology or Social Work required; licensed-eligible (clinical supervision provided).
  2. If licensed LCSW, must maintain current licensure through California Board of Behavioral Sciences
  3. A minimum of one year of experience working with HIV populations required.
  4. Experience and sensitivity in working with diverse organizations and constituencies, including the HIV/AIDS, Latino, Gay, Lesbian and Transgender communities.
